Time of Violation (in-game or replay): Towards the end, makes call to send 7 with clearly no mid and naked king and calls ME noob and blames me for not pushing when I simply try to keep us alive.
Additional Information: Not sure if this reason enough for ban but I do not appreciate this kind of behavior .

Not sure how this warrants a Ban request. You were 7/1 for 7 – holding nothing, I believe push would have helped your team out more. The call was based on OJ getting another spawn. Yes it was risky as you had no king scout.
